Abstract
We describe 27 children and adolescents <18 years of age who received bedaquiline during treatment for multidrug-resistant tuberculosis. We report good treatment responses and no cessation attributable to adverse effects. Bedaquiline could be considered for use with this age group for multidrug-resistant tuberculosis when treatment options are limited.
WHO prequalifies key treatment for children with TB (post)
01 September 2017 - WHO has just prequalified a two-pills-in-one paediatric medicine that is critical for the continuation phase of the six-month treatment required to cure tuberculosis (TB). The medicine – rifampicin 75mg + isoniazid 50mg – is a fixed dose combination (FDC) tablet manufactured by Macleods Pharmaceuticals Limited. WHO medicines prequalification activities are partly funded by the Bill & Melinda Gates Foundation and by Unitaid.

Newly published estimates on child mortality from tuberculosis paint a dismal picture. Almost 200,000 children under 5 died of TB globally in 2015, a far higher number than previously thought and enough to place TB among the top 10 causes of child mortality worldwide, according to the September 2017 issue of the Lancet. Yet, nearly all the children who died from the disease did not receive treatment, indicating a “substantial scope to reduce this burden,” the article said. Therefore, despite the currently dire situation, there is a silver lining for countries that are bearing the heaviest burden of the disease.

Childhood TB is a major issue, infecting up to one million children each year. The good news is that there are several effective diagnostic, treatment, and prevention strategies that can have a major impact on decreasing the burden of childhood TB and mortality. These include interventions like contact tracing, fixed-dose medication, and rapid testing. We must consider however, that not every strategy is going to be equally effective in every context. What works well in Kenya may not be as effective in India and vice-versa. With limited resources at our disposal, how do we decide which intervention approach to invest our efforts in? How do we make sure that the options we select will have the best evidence-based impact on child TB morbidity and mortality in the country we’re working in?

Since launching in 2015, these new medicines have begun to reach children around the world, setting a precedent for future action in the fight to end TB.
